| New students
need to provide the following information before enrolling:
1. Proof of Residency: (Utility Bill with Parent Name or Owner's Name
and a letter from Owner stating that they live there.) If Owner writes
letter, then in one month Parent must have something in their name at
the address.
2. Birth Certificate OR Medical Card with Birth date OR Foster Care paperwork
with Birth date.
3. Immunization Records
___Polio - 3rd (if last dose given AFTER 4th birthday)
___DPT - 4 (One more dose if last dose given before 4th birthday)
___MMR - 2 (after 1st birthday)
___HEP B Series
___Varicella - 1 dose
___TB Test - All students entering a Riverside County School for the 1st
time, preschool through 12th grade, must obtain a test prior to school
entrance.
4. Language Survey (give copy to Edna if line 1, 2, or 3 say other than
English. Also, give her a copy of the emergency card.)
5. Student/Parent Background Survey
6. Expulsion Affidavit
7. Record of Prior School Programs
8. Blue Immunization Record Card
9. I.E.P. - Special Education (if marked speech/resource or special day
class)
10. Nutrition Services Form
11. Emergency Card
