Standard Student Registration Guidelines
Registration may be completed during regular school hours. Staff will be
on duty to answer questions.
Listed below are the items which you will need when registering your
child. |

1. |
Please bring evidence of birth date, such as a birth certificate,
baptismal certificate duly attested, or a passport. |
|
| |
2. |
State
law requires medical verification of immunization by California School
Immunization Record. |
|
| |
|
a. |
Diphtheria, Pertussis or Whooping Cough, Tetanus (DTP) - four (4) doses
-- one more dose if last dose given before fourth birthday. |
|
| |
|
b. |
Polio
(OPV) - three (3) doses (if last dose given AFTER fourth birthday) or
four (4) doses regardless of when they were administered. |
|
| |
|
c. |
Measles or Rubeola, Mumps, Rubella (MMR) 2 doses - one dose on or after
first birthday. Second dose required for entry into kindergarten. |
|
| |
|
d. |
Hepatitis B - 3 doses -- Children who have received at least one dose of
Hepatitis B vaccine may enter kindergarten on the condition that they
receive the remaining doses within specified limits. |

|
e |
Negative Tuberculin Skin Test or chest x-ray taken within one year prior
to entry into kindergarten. |
|
| |
|
f. |
Varicella - one dose or documentation by medical provider of disease or
immunity. |
|
| |
3. |
Physical examination a maximum of six months prior to entry into
kindergarten. |
|
| |
4. |
Evidence of residency. Provide two forms of verification of address
(utility service payment receipts, and lease or rent agreement receipts/escro
papers or property tax payment receipts). |

California State law requires that children be immunized for the above
diseases before they can enter kindergarten. Students shall be exempt
from immunization only if a parent or guardian complies with one of the
following: |

a. |
Submits a written statement from a licensed physician indicating the
physical condition of the child or medical circumstances relating to the
child are such that immunization is not considered safe. |

| |
|
|
Parent or guardian files a written statement indicating that such
immunization is contrary to his or her beliefs. |
|
| |
If
your child’s immunization is incomplete, make an appointment with your
doctor or county health department immunization clinic. Take existing
records with you. Hemet Health Department is located at 880 North State
Street. For appointments, please call 766-2450. A walk in clinic is
every Tuesday from 8:00 a.m. - 11:00 a.m. Immunizations will cost $10
(one or more) and an additional $10 for the Tuberculosis skin test. |

Hemet
Unified School District Health Services is offers a clinic to provide
FREE IMMUNIZATIONS and TB skin tests. Physical examinations for CHDP
eligible students are also available. Please contact the District office
for dates and times at (909)765-5100.
